There seem to be four versions of this that I am aware of:

1969 Edition by Gene Shelley of Canada, which includes issue #13

Abraxas Publicatioins (Micky Hades) offers a spiral-bound manuscript with all 12 issues (original 11 issues plus the 12th issue published posthumously), as well as a 13th issue of 30 pages published by Gene Shelley, and two additional items (ISBN 0-9688231-3-0) 240 pages

Paul Gordon: Original 12 Issue version available in perfect-bound paperback format, and in e-book format from Peter Duffie's Alternative Magic and Lybrary.com

Miracle Factory's Rusduck and his Cardistry CD. Contains the original 11 issues additional material by Rusduck from The Phoenix and The Linking Ring.
